How long do utility sensors last on battery?

Years, typically, but reporting policy dominates over cell chemistry. A sensor reporting hourly over a low-power wide-area link behaves very differently from one reporting every five minutes.

How are thousands of distributed devices supported?

By assuming they will not be visited: remote diagnostics, over-the-air update, self-reported health and defined degraded-mode behaviour, all designed in rather than added later.
